Nurse Case Manager

RN Case Managers work telephonically with chronically and critically ill patients to ensure that available healthcare resources are being used in a timely and cost effective matter.

Responsible for utilizing the nursing process in the development of treatment plans, with established goals, implementation of those plans, and monitoring and evaluation of an assigned case load of patients in order to provide them with the quality care appropriate to meet their clinical needs.

Qualifications

* Associates degree in Nursing or RN Diploma required. Bachelor’s degree highly preferred.
* Two (2) to four (4) years of clinical experience, preferably in Case Management or Home Health Care with a Medical/Surgical background. Discharge Planning experience.
* Registered Nurse with current Michigan Registered Nurse license required.
* Certification in Case Management preferred. If hired, CCM certification will be required within one year of hire date.
* Prior experience in Telephonic Outreach and Counseling preferred.
* Demonstrated clinical knowledge and experience relative to patient care and health care delivery processes.
* Working knowledge of Case Management principles and procedures based on nationally recognized standards of Case Management.
* Excellent analytical, organizational, verbal and written communication skills.
* Ability to work independently and in a team environment.
* Ability to be flexible and to manage multiple tasks.
* Good working knowledge of PC applications.
* Other related skills and/or abilities may be required to perform this job.

Work from home after training is complete and case load can be managed successfully. Home office and computer requirement(s) will be provided during interview process.

Required Hours: 10:30 a.m. to 7:00 p.m. 2 days a week. The start time for the remaining days can flex from 8:00 a.m. to 9:30 a.m. If working more than 8 hours, can start as early as 7:00 a.m.
